Appointment of Manager

This form is filled out by and authorized officer of the Owning Company or the Owner himself if it is an individual, and notifies this Administration that a manager has been appointed for a ship. This fulfilled the Requirements of the ISM Code Section 3.1 and a copy can kept on board as evidence if required. Once completed the form should be submitted by e-mail to registry@bermudashipping.bm
