This is the new version of the Application for a Safe Manning Document in .pdf form.   This version requires the applicant to propose a manning level with justifications where necessary and also allows for the issue of multiple Safe Manning Diocuments when a vessel is likely to alternate between different modes and this is appropriate.

Form 2 is to be filled out by the master of a Bermuda registered ship in order to inform this Administration that some details of the CSR require to be amended.  Once the form is completed it can be submitted electronically to   This e-mail address is being protected from spambots.
